Scheduling a Medical Marijuana Evaluation Appointment
If you qualify, please make sure that you have with you at least two recent medical documentation of your condition(s) and a valid photo ID as a WA resident (driver license, etc.) We recommend you stop by your physician's office, clinic, or hospital where you received medical treatment and request a copy of your last two visits. This will help you avoid unnecessary expenses of copying irrelevant chart information which can also delay the process for a week or more.
If you need us to get your records, please fill out our Medical Records Release Form
and send it to us so we could request the records for you. For more information about what you need to bring to the appointment and what medical documentation is needed, please contact us.
Once you feel you have all the proper documentation and would like to book your appointment online, please follow these simple instructions.
To make an appointment:
1. Click on this link: Book your appointment here
(You will need a Google account)
If you already have a Google account, log in. If you do not have a Google account, you will need to sign up for one.
Once signed in
2. Pick a day and time slot that is available and click on the proper "New Consult" button. Each time slot is 10 minutes long.
3. A box will appear to book an appointment.
In the "what" area, type in your first and last name. In the description box, please provide us with your telephone number and any message you would like to send us, so we can contact you for further information about your apointment.
4. Then click save. Once saved we will receive an email that will let us know that you have booked your appointment. After that we will contact you shortly during our normal business hours to provide you with more information and to confirm your appointment time.
