Appointment
To avoid any delays, please familiarize yourself with our appointment process:
- Before we can schedule you, we will need medical documentation of your condition(s). You can send it to us via fax,
e-mail, or mail. You can also fill out our medical records request form and send it to us so we could request the records
for you. For more information, visit the FAQ page.
- When you arrive for your evaluation, please bring Photo ID and a filled-out intake form .
- Your evaluation will take about about 25 minutes, so please plan accordingly.
- After the evaluation is completed, our physician will review the information and determine eligibility.
- You will be notified of the results immediately.
- Payment is expected at time of service.
