Who gets approved for medical marijuana?
Medical marijuana is approved for patients diagnosed with conditions that are debilitating or terminal and are poorly responsive to conventional medical treatments. For more information please see our conditions list.
How old do I need to be to qualify for medical marijuana?
You must be at least 18 years old.
How can I make an appointment at NWMCA?

What do I need to bring to my appointment?
You will need to bring a government-issued photo ID (driver's license or passport) and documentation of the medical condition you are suffering from.
How much medical marijuana can I possess as a qualifying medical marijuana patient?
You are permitted to possess no more than 24 ounces and 15 plants.
What kind of medical records are needed?
The needed medical documentation could be your last two chart notes, imaging reports (but not actual MRIs or X-Rays), preferably dated no further back than 3 years. Also, no more than 10 pages.
How can I obtain my medical records?
Fastest route is to go directly to the medical facility where you last received treatment and request a copy of your last chart note to bring with you to your appointment. Alternatively, you can use our medical records request form to allow us to assist you with this. Please remember to leave your contact information on any documentation you send us.
Where do my medical records need to be sent?
They can be sent via fax at (206) 407-2577, e-mail, or mail to 570A Industry Drive Building 6, Tukwila, WA 98188.
Do you share my medical records with anyone?
We do not share your medical records or any of your personal information with anyone.
How long does a physician's recommendation last?
NWMCA physician's recommendation is valid for one year.
What kind of payments do you accept?
We accept cash and credit cards (VISA or Mastercard).
